Free Activity Waiver Template (PDF)

Organizing a guided tour, outdoor activity, workshop, or equipment-based experience? An activity waiver can help explain what participants are doing and document the information they have been given. It is particularly useful when a group will be using bikes, kayaks, sports equipment, or other gear supplied by an organizer.

This is a supporting document, not a rental agreement by itself. Use it alongside an agreement that sets out the terms.

Why use an activity waiver?

Participants may have different levels of experience, and an activity can involve conditions that change during the day. A written document creates a shared reference for the activity, participation rules, and acknowledgments. However, a signed waiver does not automatically eliminate an organizer’s duties or liability. How waivers are treated varies by jurisdiction and by the nature of the activity.

What to consider including

  • Name of the activity, organizer, location, and scheduled date or period.
  • Participant identity and suitable contact details.
  • A plain-language description of relevant activities and foreseeable risks.
  • Any participation requirements, safety instructions, or equipment rules.
  • The acknowledgment, consent, and signature sections appropriate to the activity.
  • Separate handling of minors or guardians where local rules require it.

A checklist to think through for your situation. It is not a list of clauses: the blank PDF’s own sections and lines are listed above.

Before the activity

Explain the important terms rather than assuming a signature proves everyone understood them. Give participants time to read and ask questions. A group signing flow can simplify collection when several people are joining the same event, but the document still needs to fit the activity and local rules.

FAQ

Questions

Does signing a waiver mean the organizer has no responsibility?

No. A waiver is not a universal shield against responsibility. Its effect depends on local law, the wording, the people involved, and what happened.

Can one waiver be used for a group event?

You can use a suitable waiver for a group, but participants may need to provide their own details and signatures. Rental Agreements supports group agreements that people join through an invite link or QR code.

Can I sign an agreement without printing it?

Yes. Rental Agreements lets you complete an agreement and sign on your phone or request a signature by email. The recipient can review and sign in a browser without an account or app.
